Check out Scarab at http://scarab.tigris.org. We've been using this product in house for a couple of months now and we are very impressed with its capabilities.
Another one worth looking at is BugTracker.NET (I'm the author). Especially for folks who are more comfortable with Microsoft tools. It's sort of a like a poor-man's FogBugz.

For anyone out there in doubt of the value of a dedicated bug tracking system, why not just try it out with a leightweight solution such as 16bugs.com or BugWiki.com? Both allows you to get started in seconds with no installation, configuration or learning to do.
I'm using XStudio (http://www.xqual.com). We started to use this just as a test management tool (this is its main goal) and it turned out that it's very good to have the bugs managed into one single framework.
Some people in the QA team use it only as a bug-tracking tool.
It's free and we are quite happy about it so far...

Just signed up for the free account to give it a shot. We are looking at it and a few others. Even if we don't end up using it, just wanted to say nice design!
We used to use Github and Bugzilla but eventually bit the bullet switched to Assembla (http://www.assembla.com). We found their bug tracking tool was best for us and it turned out to be a lot cheaper than we imagined.
