Don't deliver what the customer wants is a consequence of failure, not a reason for it!

Why didn't the customer 'get what they wanted', because for many reasons, many involving management, if only of expectation, we failed.....

It was you, no it was him or them or it, or Mr Nobody. While we don't understand our contribution to failures, all other failures are irrelevant!
