Define Success
The real problem failure occurs is because success is not defined properly. A developer thinks success is having the module compile. A project manager thinks success is meeting the deadlines. A user thinks success is having a new system to use.

But to a business, success is solving a problem. If you aren't willing to take the time to clearly define the problem, then you cannot clearly define success and it gets redefined as whatever the scope of the employee's involvement is.

It is a management problem. It is easy to spend money. It is easy to develop software. It is easy to meet deadlines. What is hard is solving a business problem, especially if the business managers cannot, or more accurately 'will not,' clearly define the problem.
