I agree that it is not always the boss. I have surpervised and managed and there are certianly employees with issues. I would give a 360 review so that I could get feed back from reviews as well. This helps to figure out if it is the boss or the employee.

I have never left a position due to a bad boss either. I have left various jobs for more money and a better position. Currently it is due to the boss.
