You bet I have!
How about this: the boss puts you and your team in an impossible situation, where you have to deliver a product based on zero information and zero cooperation. So you warn him this is a disaster waiting to happen - on several occasions - then start to invent the best guess you can of what the client wants.

Six months later, one of the components delivered is so far off the mark that the client yells at the project lead, who yells at me and blames me and my team for gross incompetence - while my boss looks the other way and pretends it's nothing to do with him.

And is duly shocked when I hand in my notice.

Stay in a place like that, and you end up going crazy.
