Choke Points
Although I agree that having 'gates' or project reviews where the status and risks are openly evaluated, the two challenges I see with choke points is that optimism almost always reigns supreme, i.e., the belief that things are going to get better (productivity, technical issues, etc.) and that the decision process is working from incomplete information, usually due to a lack of completely documented and understood requirements. So the organization presses on,living on "Hope Island"
