MBam / ComboFix / TDSSKiller
It's been mentioned above, but here's my one-two-three removal process for about 90-some percent of infected PCs:
1. Malwarebytes
2. Combofix
3. TDSSKiller
If they can't remove it (even in safe mode) then I usually have to yank out the hard drive and connect it to a working PC to do some further cleaning.
Those three tools are wonderful. I've stopped using Hijack This, Spybot, and SuperAntiSpyware in favor of those. They save me (almost) every time.
My favorite free antivirus lately is MSE. I think it had problems when it was first rolled out, but now it seems very stable, and light on resources even. Someone made the comment that MSE is a resource hog, and I haven't seen that at all. In fact I usually see a performance increase after removing something like McAfee or Norton 360 and replacing them with MSE. I'm not a M$ fanboy, I just think it's a solid product.
