The questions we ask define the types of problems we will solve. Thanks so much for this list. Here's one more question that I always ask at the end of a key decision making meeting AFTER we've created a list of What To Do, By Who, By When.

Ask: If we take these actions by the dates listed, will we be addressing the original issue defined at the beginning of the meeting? So much so that we're very confident that a good portion (more than 80%) of the pain of this original issue will be resolved?

If the answer is NO, then there's something that was missed. Either the right people weren't present for the discussion, or sometimes there's a sacred cow that no one was willing to call out to address the real problem. Or the answer is no, because we lost sight of the original issue we were trying to solve, or worse, didn't have agreement on that at the beginning.
