The only time I've ever needed such things to fix a system is when I inherit one at work that's had a lot of this very stuff put on it in the first place. That or they've been going to toxic websites and doing things they shouldn't have.

My own computers have always been fine with just basic anti-malware protection, and now that's MS Security Essentials.
