I believe that there is also a conflict between a process (or a service) owner and the process (or service) manager as the manager is in charge of operating the process (or the service) and the owner who is in charge of the effectiveness and the efficiency and to propose some improvements. Obviously this can conduct to a conflict between the two roles. However in small entities the two roles are often assigned to the same person.
I have had good experiences when the release manager was also heavily involved with test, if not the test manager. These seemed more synergistic than you suggest.
The idea of getting someone else to proof-read your work well and truly predates ITSM and ITIL, and with good reason.
Jim, I think everyone would agree the resource charged with providing Service Validation & Testing as well as Evaluation should definitely work very closely with the RAD Manager. I'd go so far as to say that such a relationship is essential. But that's a very different kettle of fish than suggesting that those two roles be invested in the same person.... my own observations from experiences within several different process areas is that it's just a Bad Idea(TM).
