I've had number 4 happen.

A fairly senior programmer took on maintenance of an in house VMS to IBM mainframe comms program. After his first mod he brought down mainframe comms for our whole site so it was all hands to the pump to try and get things going again.

Doing a diff between his source and the previous version showed tonnes of code missing. When asked about it he said "I didn't understand any of that stuff, it looked weird, so I took it out"....

At least none techie users have an excuse for this kind of stuff!
