Just a matter of resource reality
We don't have a 'CIO' per se as we're only about a 160 employees. We actually have SA on our 2003R2, so the only thing holding us up from moving to 2008R2 is actually me as it's on my 'to-do' list to do the upgrade and I just haven't had the time.
That type of issue is part of my reasoning why I disagree with a lot of the TR articles because they don't seem to understand how reality is in a business where there's an IT Manager, a Network Administrator (me), 2 solely dedicated Developers. I handle everything from the basic Tier 1 stuff up to intermediate SysAdmin stuff, AD/Exchange account management, etc. We have nearly 400 PCs I maintain, over 20 networked Laser Printers, plus cabling, station moves, wireless network maintenance, on and on.
CIOs need to come work 'in the trenches' and see how reality really is.
