I have been involved in a number of projects where solid project management and execution was in place and the only thing that made the project 'fail' is that some upper management guru up and decided that they didn't need it after all, or the enterprise was going belly up because of unrelated issues, or some other reason beyond the skill of any project manager save those with some sort of clairvoyance.
(In some cases you actually can see such 'out of the blue' bolts coming - when 'strange' things start happening, but often it is unrelated to anything that makes sense.

PM101, well said ! Even if they would not teach anything else except this list on PM101 it would help a lot - it was the first thing I did learn on IBM training 1971! Now, after 35 years on computer projects, I seems to me that PM teaching / training stopped sometimes late 80's. Our current PM's couldn't even understand the terms on this list, most of them 'hackers' from 80's or programmers (not developers!) from 80's and 90's or, maybe worst, young salespeople who never made it in sales / marketing. I would add one or things to the list, seem obvious but.. How to get the requirements and when to freeze the current phase and when to start the next.
There seems to be a true lack for project management knowledge or enforcement. There are many reasons for IT failures. Many can be avoided or minimized but unfortunately there is excessive waste and mismanagement in the public sector due to political careers, elections, cronyims and political appointees. Billions have been needless spent and I am confident much of this is do to the fact that individuals are positioning themselves for careers outside of the public sector. The federal government should have an investigation unit that crosses jurisdictions and has the power to posecute those individuals for mismanagement and corruption. Unfortunately resources are limited and waste continues to sky rocket and the taxpayer is the victim of the pillage and plunder mentality.
I missed you missed the most important factor: Lack of stakeholder support. I think this article ( http://www.pmhut.com/why-projects-fail-2 ), in conjunction with yours, creates a better compiled list of why projects fail.
Actually, I still believe most of the tools I see and read about online everyday are mostly under deliver, but yet, there are some tools which I believe are worth the time I spent on researching them. Project Tracking Software http://www.replicon.com/time-bill
