You hit the nail dead-on.
However, the people making these mistakes have lost their perspective. They are clinging tight to their "methods" and their habits they carry out in the name of "security."
Our IT staff would just as soon lock the door to your office 24/7 and call that computer security. I exaggerate... but not by much.
