After several more tries, I got the file to download as a zip instead of a word document. It had a PDF and a DOC version in it. The PDF version works fine, but the DOC version is still no good.

After reviewing the file, I can say that it looks like a very good survey. It covers a lot of the most important aspects of how we communicate and work with clients. It will be helpful to me.
