Our definition of issues does not necessarily mean a problem. An issue is something that needs resolution so the project can proceed; it may be a problem that needs to be fixed or it may simply be a decision that needs to be made.

We also track issues that can be resolved by the project team or pm. So an issue does not imply resolution above the pm level.
