wanted - a PHP based note-taking tool

hello,

just in case anyone has info - I would need a tool (preferably PHP) to run on a webserver, and which can be used for note-taking.

example: a tool that I could use for reporting/summarize my 8-hours of work today...what I did, didn't do, issues resolved / issues pending etc.
