Application Architecture for cloud deployment - What is the best approach?

I have been trying to identify the best possible approach to migrate existing applications to the cloud. At the same time the approach will also include tenets that are applicable to architecting new applications for cloud deployment. Basically if I want to become a SaaS vendor and make my existing applications available as Services using either a Private / Public cloud ... what should be the architecture of my applications to be successfully on the cloud and to work in the best possible manner (optimal performance/ resource utilization / cost effective ... ).

Is there any reference architecture that may be followed ? What if I take my existing application and put it on a cloud - will it work, if yes how do I know that it is making optimal use of the cloud and more importantly how do I know whether it is working optimally ?

The best approach identified so far includes - design to scale on the cloud (making use of the massive scalability of the cloud environment), design for fault tolerance, include management and monitoring software components.

Yet another appraoch suggested is to use a Cloud Application Platform that facilitates the rearchitecting of applications for deployment on the cloud environment. Also it allows the application to make optimal use of the cloud resources.
