Customer Satisfaction Survey

I manage an IT Department with 8 locations, and approximately 400 nodes. We are looking at possibly revamping out IT support, and would like to survey our users to determine our current level of effectiveness, and to measure user satisfaction. Do you have any templates?

In addition to the links from the previous post on actual surveys, you will want to make sure that you have both graded and subjective questions for your end-users. Be sure to put the graded questions in a spreadsheet so that you can track your success/failure rate over time and apply it against any other changes that may be happening. The subjective questions are good to have because it allows your end-users to add items that you may not typically find in a graded question.
