Form fields in headers

I want to create a template form (That's the easy part) However, this form has a header that needs to be filled in each time too. Is there a way to use a form field to fill this since you can't put a form field directly in a header?

You can write a VBA Macro. You can get the required data using inputbox or any other appropriate method. Create strings and write those strings in the header or footer using the macro.
As an example the following simple macro created by recording types 'This is it' in the header.

You should be able to make the form field a bookmark and reference the bookmark in the header. Is the value you want in the header entered by the user? If yes, I think the bookmark will work. lo

Took a little deciphering but thanks it works great. Actual terminology is a cross reference field in the header.
