Hdd goes offline

I am using Intel 2U Rack mount Barbera server.The config is Intel LB440gx mbd,P3 550cpu,128mb SDRAM.Installed Raid card Mylex Accelraid 170 single channel.4 hot swap IBM 18gb hdd used.OS WINNT 4.0 with SP6.
MY question: Hdd goes offline intermittently.It may be one or two hdd ,we have to reset ,manually make hdd on line and rebuild.
Any answer?

Apparently time-out errors are not unusual with this family of controller in the intel 440bx chiset motherboards.

Q:A Mylex AcceleRAID 250 controller installed in an N440BX server system with an Astor chassis. The system appears to be running without any problems, but the event log is showing a few errors "The device device/SCSIport 0 did not respond in the timeout period". How do we get rid of these messages?A:An occasional timout error message in NT is typical - even the best systems sometimes get a glitch which the driver recovers from, noting the event in the log. If the error frequency increases dramatically, or you start seeing any other kind of instability, this is an indication that a problem has developed, such as a device about to fail, or a problem with the cabling or termination.

Also, check the cabling and termination to make sure it's firmly seated and configured correctly, to reduce the probability of getting these errors.

amazing how those who don't support a production machine feel that an occasional time out error is acceptable...Go Figure....

Lots of HDDs are having problems with their firmware that makes them offline. I know this problem from several HP Netservers, where you need to update the firmware of the HDD (all HDDS should have the same FW release) and then everything is running well, without any offline problems.
E.g. HP works with IBM, Seagate and some other HDD manufacturers, and nearly all need an update, also when the hdd is quite new. You should contact IBM webpages or mylex, as I dont know wheather the HP firmwares for this disc will help u. In the case you want to test it, you need to go to hp.com - drivers - enter "firmware" at the search field - go - and take a look. FW update for a HDD works like an Bios update for a motherboard, you need to create a start disc (normally ther should be a tool on the manufacturers webpage that create a startdisc directly starting with the update tool). Should be working fine, as I know.
When not: Cmos clear of your machine and Bios update could help too, but dont forgetto save or write down your bios settings before !

To some extent this option is ok.
In my case all the fw updates has been done and still problem persists.
