Has anyone developed a questionaire for assessing the quality of services provided by the IT department to internal customers?

I have a survey that I would be able to share with you that comes from Gartner TCO software modeling tool. I will send you the file and it is fairly complete. The main thing to do is to be sure to think hard about the first survey because in orderto ser a baseline you need to be able ask the same question at least once a year.

I can email you ither examples that are more in depth. My advice is to keep your questions aligned with your goal as an IT support provider. This is a little messy but I can send it as an attached file if you contact me.
