If the code works well, it may not need so much documentation.

Depending on your methodology (e,g, Agile), documentation may be a low priority thing.

Part of your application development standards should specify what artifacts, such as documentation, are required at each step of the process. If you have no standards, that's one issue, and if your people are not following standards, that's a different issue.
