One of the simplest things is also the one that derails many IT transformation efforts: the ability to make decisions. Whether it's because people are afraid of hurting feelings or that they just want their say, many organizations are now driven by a consensus culture. No decision can get made unless everyone agrees or at least has had their say. This leads to what the authors refer to as consensus paralysis. The decision making process slows to the point that, in effect, nothing really ever gets done. The obvious impact is that much needed change is stifled. But even more damaging, consensus paralysis leads to apathy, cynicism and disenchantment within the organization as the team learns that nothing ever changes.
