Eastern and Coastal Kent Primary Care Trust (PCT) is a healthcare provider in the United Kingdom. The PCT wanted to reduce unnecessary hospital stays for elderly patients, but needed a way for multi-disciplinary care teams to build integrated care plans for each patient. To improve collaboration and ensure adequate home care for patients, the PCT used Microsoft Office Groove 2007 software to create a "Virtual ward" where patients can be treated and monitored at home.
