Production Reject Report

Use this template to record the occurrence and reasons for any rejects. You may want to add a column for the production line on which the rejects occur in the event that you have more than one production line. Similarly, you may want to use separate templates for different products. This template is designed to be filled out by hand, though it can be filled out electronically.
