Looking for a generic plan for data center move

Wondering if anyone has a generic data center equipment move plan that I could revise as necessary to reflect the specifics of my situation. Scope would not include any construction requirements of the new site other than certifying that the required environment (electrical specific to the moved equipment, cooling capacity, etc.) is in place and addressing any identified gaps. This is not a move into virgin space. I'm looking for something that covers the waterfront, including: (1) certifying new site readiness, (2) application systems dependencies, (3) network cutover, (4) move day & go live process, (5) risk assessment and fallback, (6) disaster plan revisions, (7) etc.

May I suggest the first step is to.... run for the hills. DC relocations are not for the faint hearted. However if you really want to continue a good start is http://www.infosys.com/offerings/IT-services/information-management/white-papers/Pages/data-center-migration.aspx

This can be found in the realm of BICSI. Covers this and more. When I manage data center moves or migrations I start with the apps first, then synch all that up with the inventory of the machines on the floor. At that stage I'm working with the in house Sys admins to plan migrations based on the criticality of the app. Big note on power and cooling.
