It is hard to achieve, because the complexity of software tends to be high. Software failures may be due to errors, vagueness, omissions or misunderstanding of the specification that the software is supposed to satisfy, inaccuracy or inability in writing code, inadequate testing, improper or unforeseen usage of the software or other unforeseen problems.
