Access Question

It has been a long time since I worked with access, but this is what I am trying to do.

I have an excel spreadsheet that I linked to in Access. The spreadsheet contains duplicate information except for three fields. Here is what I have:

Fn Mn Ln DOB Date ENCode LVCode

I what to be able to list all names, single, with all the dates, ENCode, LVCode.
