agreement form for network services?

does anybody have a lead or info on attaining a generic “agreement for network management services?” we’re in the process of providing support and management of customer’s in-house network(s). i’m supposed to come up with some type of agreement for the customer to sign. any type of form would be appreciated. thanks.
