APPLICATION SOFTWARE REVIEW

I urgently need either Systems or Application Package review documentation templates and user Specification/Requirements guidelines,documentation or templates.We have been using a system for 4 years now (initial user requirements document not available) and would like to see if it still meets organizational goals, any body knows? where would i find these?
