Assessments & Surveys – Tech in schools

I was wondering if any one could lead me to resources for developing assessment and survey tools and instruments for technology in K-12 school environment. I am consulting on a project with 2 large school districts – with up to 50 schools = yikes. The goal is to determine what technology the schools have and need – infrastructure, workstations, labs, etc. – and what tech aptitude the teachers and admins have toward making it all work for the benefit of the students. ANY LEADS IN THIS AREA WOULD BE DEEPLY APPRECIATED. Aside from additional studies, I specifically need to know if there are any models or templates i can refer to. I would be grateful for any help here.
