CRM Start-up

I?m new to CRM concepts and I?m trying to build a problem/solution database for my customers, but I don?t know where to begin. What should I ask to them about their incidences? I think that I have to ask not only technical questions, instead, I?m interested in the quality of our service and the perception of our company to them. Any ideas of questions to ask?
