Frontpage 2000 database question

I have used Frontpage 2000 to create a form that is suppose to be tied to a database, and in turn viewable on the web.

you can see when you submit the form, it looks like it took it, but it doesn’t show up in the access database like it should and doesnt show up on the results page at:

https://www.acallforhelp.info/local/os007/portal_1/portal/test/results.asp

any ideas? I know nothing about scripting and stuff … trying to do it quick and easy with Frontpage.
