Help Desk Survey

We are using Peregrine 4.07. I want Peregrine to automatically send a survey to a customer. Is there a way in Peregrine to send a survey to a customer every 4 times we close a incident for that person. Can you actually create a survey in Peregrine as well or do you use a third party software (i.e. zoomerang). Also, looking for sample surveys to get us started with the actual questions.
