How do I create a survey in Outlook?

I am trying to create an answerable survey usng option buttons in Outlook 2000. I get to the create new form section and am unable to enter my questions on the design window. I know I am missing one crucial step but I cant figure out what it is. I’ve tried Help and Microsoft without results. Any ideas?
