How to Evaluate Good Employees

I have got a specatacular IT staff who support executives and Administrators at a state university. We have evaluations coming up and I would like to survey those users we support to gain their opinions on topics such as work quality, knowledge, andcommunication. What formats are generally available and advisable to do this? What thoughts do members have on this?
