interactive web pdf forms

Problem: I need to create a fillable front end pdf form that will be translated (both form info and input info by the user) and then created for download by user.

Thus far, I am drawing a blank. Mind you the information being input will be general personal information such as: address, phone number, country of birth, D/O/B, and the like. The rest of the information is of the “yes/no” variety.
