Keeping Track of Laptops

Question: Does anyone have a sample Laptop Responsibility Agreement? I’m looking for a model to use for employees to sign when they are given a laptop. The doc would describe the laptop, and outline the employees responsibility for care. For exemple, it would state that the employee has to replace if it is lost or stolen?

Any ideas on where to find a model? Thanks!
