Logging network, e-mail, phone changes

Hello. I was wondering what methods fellow members use for network admin changes (IE: network accounts, e-mail, phone/voicemail, access cards, etc.)? A description of process would be helpful too. We have used paper-based logging forms & Excel spreadsheets. I’m considering the use of web forms & exporting data to Excel, or perhaps using a Access database. I’m just wondering what works well for others.
