Network/Server Down Time tracking

I have always tracked server and network down time with a basic Excel spreadsheet. Now I need to find a utility or program that I can use to enter and report on down time issues. Things like what server, why, what fixed it and for how long. [with reporting features] Has anyone used or known of a product like this>
