Policies for Network Access Changes

I’m trying to put together a policy document as a guideline for network admins to follow regarding the creation of new accounts, file access rights changes, etc. I have several admins performing these functions and would like to standardize the procedures to ensure uniform management of accounts. I would appreciate any sample docs or site referals. Thanks!
