Pre-intranet user surveys

I have been asked to develop an intranet for my company. As I am still in the initial stages of the design and research, I am creating a questionnaire to send to several users in each department to solicit their input. Does anyone have existing surveys that they used for their intranets or know of any resources I can go to find samples of such surveys?
