QM Governance / Enforcement & Penalties

Hi, seek advise on implementation QM matrix. What is expected is to set clear guidelines on how QM is to to adoptted in projects & sdlc and a mechanism / rules / matrix that tracks relevent induviduals and score such that the guidelines and sets of expected outcome sort of becomes a balance score card with relevent expectations becoming Key Performance Indioators (KPI).
