Risk Database

Hi All:

I was wondering if there is an off the shelf risk database that could be used to store/log/update the risk as they are identified/mitigated/closed through the project. I am currently using an access program wherein i have used some settings set for probability/severity of the risk.

Would appreciate your input on how you are tracking risk on your projects.
