Survey for internal staff to know their ERP software requirement

I’m currently working on ERP solution. I need to create a questionaire / survey for my company’s staff to fill in their requirement / expectation from the ERP software.
What question should i ask and who should i target? (eg: ppl from Sales, Purchasing, Store…)
