Technology Plan

I am looking for a good template for a “Technology Plan”.

Definition of a technology plan:

a) document which states a “high-altitude” operational goal for a business or company in regard to it’s networking and data services

b) illustrates how to accomplish the stated goal using specific technologies and practices.

Any takers?
