USB / CD hangs or freezes WinXP SP2

I have a system that about half the time when I plug in a USB device, or load a CD/DVD into the tray, Windows XP completely locks. The only way to get back in is to do a hard reboot.

I have reformatted and reinstalled the O/S, verified drivers, even got a different USB hub thinking that might be bad, but the problem occurs even when I don’t use the hub and plug in directly.

All my devices work without issue on my other PC.
