User Account Auditing

Does anyone know of a single tool that can collect user accounts from multiple platforms like NT, W2K, Unix, MS SQL, VB Apps, Oracle, SAP, etc?

I looking to try to simplify the administration of user accounts in several different platforms and applications into a single interface or even report.
