web and exchange behind proxy server

as follow the KB Q181420 “How to Configure Exchange or Other SMTP with Proxy Server”, I can receive mail behind proxy server, but I can not sent mail out through proxy server (I did not know where is the mail goes), and how can I forward web requestto my IIS (internal IP) that behind proxy?(web request only reach my IIS (external IP) that installed with proxy server)
